Bethel Congregational Methodist Church

We are excited to offer a theater camp for K - 5th graders.
The camp will be held Tuesday, May 28 - Friday, May 31 from 10am - 3pm each day and on Saturday, June 1 from 11am - 5pm.
The performance will be Saturday, June 1 at 6pm.

Kids must have completed K - 5th grade.
Kids attending the camp must bring their lunch each day, snacks will be provided.
Dinner will be provided for the kids on Saturday before the performance.
Shirts are included in the registration fee. Must register by May 15 to be guaranteed a shirt.
Spots are limited to 30. There will be a waitlist. Registration is first come and confirmed with payment.

I give permission for my child to attend and fully participate in the above event under the supervision of representatives of Bethel Congregational Methodist Church, and for said representatives to obtain any necessary medical attention in the event of injury or illness. I verify the provided information to be correct and release Bethel Congregational Methodist Church and its representatives from any claims arising out of damage or injury sustained during the event or any activity pertaining to the event.

Photo Release Form

I hereby grant permission to Bethel Congregational Methodist Church to film, video, and/or audiotape recordings, slides or photographs to be taken during regular programming and special events that occur at Bethel Congregational Methodist Church or off-campus venues for Bethel Congregational Methodist Church-sponsored events. I understand that this media will be produced and used for promotional purposes of the ministries of our church. I authorize Bethel Congregational Methodist Church to use my minor child’s images on their World Wide Web site, social media pages, or in other church printed materials without further consideration, and I acknowledge the right of the church to crop or treat the media at its discretion. I also acknowledge that the church may choose not to use my minor child’s image at this time, but may choose to do so at a later date at its discretion.
